Job summary

UnitedHealth Group's Optum Care at Home is seeking a Telephonic RN Case Manager to serve as primary contact for members with complex medical, behavioral health, functional, and social needs, coordinating care across Medicaid LTSS, home health, and primary care.

The role requires active RN license and 2+ years of RN experience, with strong telephonic assessment and EHR documentation skills. Remote work is available for WA or CO residents, with occasional in-person meetings as needed.

Qualifications

• Active RN license required.
• Minimum 2 years of RN experience.
• Experience coordinating care for patients with complex or high-risk needs.
• Experience with Medicaid LTSS or home- and community-based services.
• Proficiency documenting in electronic health records and care management platforms.

Responsibilities

• Conduct telephonic assessments of members’ medical, behavioral health, functional, and social needs.
• Develop, implement, and reassess person-centered care plans.
• Coordinate care across Medicare/Medicaid, primary/specialty care, home health, and community programs.
• Identify barriers to care and connect members with resources.
• Document assessments and outcomes in designated systems.
