Job summary

CareSource Management Services in Massachusetts seeks a Clinical Care Manager (RN) to provide monitoring, follow-up and care management to dually eligible enrollees with complex medical, behavioral and social needs in community settings.

You will coordinate with primary care, specialists and community resources, lead an interdisciplinary team, and promote preventive care while supporting patients in living in their preferred location.

Qualifications

• RN licensure required in Massachusetts.
• Experience in care coordination or care management with dual-eligible populations preferred.
• Familiarity with Medicare/Medicaid programs and community resources.

Responsibilities

• Engage enrollees in home and community settings to build a complex care management relationship.
• Coordinate care among primary care physicians, specialists, and community resources.
• Lead an interdisciplinary care team to develop holistic care plans.
• Oversee utilization of long-term services and supports to allow patients to remain in their chosen location.
• Provide education on benefits, services, and preventive care.
