Job summary

CareSource in Massachusetts seeks an Integrated Care Clinical Manager (RN) to deliver in-home and community-based care management for dually-eligible enrollees with complex needs. You will coordinate medical and social services, lead an interdisciplinary team, and drive care transitions to improve outcomes.

Education requires an A.S. in Nursing and an active RN license; Medicaid/Medicare experience is preferred.

Qualifications

• Associates of Science (A.S.) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs required.
• Prior experience in care coordination, case management, or working with dual‑eligible populations preferred.
• Medicaid and/or Medicare managed care experience preferred.

Responsibilities

• Engage enrollees in their homes and community settings to establish a complex care management relationship.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and social needs.
• Coordinate with primary care physicians, specialists and community organizations to ensure seamless care and avoid duplication of services.
• Assist members in accessing community resources including housing, transportation, and food assistance.
• Educate members about benefits, services, chronic condition management, medication adherence, and preventive care.
• Follow up after hospitalizations or health events to ensure continuity of care and prevent readmissions.
