Job summary

CareSource/Commonwealth Care Alliance (CCA) in Massachusetts seeks an Integrated Care Clinical Manager. This community-based RN provides monitoring, follow-up, and complex care management for dually-eligible enrollees with medical, behavioral, and social needs.

Qualifications

• A.S. in Nursing from an accredited program is required.
• RN with ability to independently serve people with complex needs is required.
• Case management experience or care coordination preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical Field/Community Based Training a plus.

Responsibilities

• Engage enrollees in homes and community settings to build care management relationships.
• Lead the interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources to avoid duplication of services.
• Educate members about benefits and available services under Medicare/Medicaid.
• Follow up after hospitalizations to ensure continuity of care.
