Job summary

CareSource is seeking a Clinical Care Manager in Massachusetts. This role is a home- and community-based RN-led position focusing on monitoring, follow-up, and care management for dually-eligible enrollees with complex medical, behavioral and social needs.

You will lead an interdisciplinary team, coordinate with providers and community resources, assess needs, and support care plan development and implementation to improve health outcomes and quality of life.

Qualifications

• Active nursing license in Massachusetts or equivalent
• Master's in social work or mental health counseling with independent license required
• Minimum 3 years RN/BH clinician experience; 1 year with 2 years complex needs preferred
• Experience with dual-eligible populations and care coordination preferred

Responsibilities

• Engage enrollees in homes and community settings to build complex care management relationships
• Serve as liaison between providers, resources, and enrollees for seamless care transitions
• Perform Comprehensive Assessments and functional/risk assessments in a timely manner
• Lead ICT to develop holistic care plans addressing medical and non-medical needs
• Coordinate with community organizations to ensure care continuity and avoid duplication
• Educate members about benefits, services, and chronic condition management
• Follow up after hospitalizations to prevent readmissions
• Travel to member/provider/community visits as required
