Job summary

CareSource is seeking a Clinical Care Manager for Massachusetts to monitor and manage care for dually-eligible enrollees with complex medical, behavioral, and social needs. The role emphasizes integrating health services with community resources to improve health outcomes and quality of life.

The nurse will conduct assessments, develop care plans, and coordinate with the interdisciplinary team to ensure continuity of care across settings, including transitions from hospital to home.

Qualifications

• Associates of Science (A.S) in nursing or Master’s in social work/mental health counseling and license required.
• Experience in care coordination or dual-eligible population preferred.
• Knowledge of Medicare/Medicaid programs and community resources essential.

Responsibilities

• Engage with enrollees in homes and community settings to build complex care management relationships.
• Lead interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with providers and community resources to ensure seamless care transitions and avoid duplication.
