Job summary

Option Care Health in Austin, TX seeks a Clinical Transition Educator to deliver clinical education and training for patient transitions to our care delivery model, partnering with the sales team to grow patient admissions to OCH services.

The RN-qualified candidate will educate patients, families and facility staff about in-home and alternate-site infusion, conduct assessments, protect patient confidentiality, and document education using our applications and technologies.

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years healthcare experience.
• Ability to travel locally 100% to care facilities.
• Strong relationship-building and customer service skills.
• Proficient in standard office software (Word/Excel/PowerPoint).

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff about how OCH services will be facilitated in-home or alternate sites to ensure successful transitions.
• Conduct patient assessments to determine viability to join the OCH care delivery model.
• Partner with the OCH sales team to identify clinical education needs and improve processes to increase patient transition volume.
• Maintain confidentiality of patient and proprietary information; observe legal guidelines.
• Provide proper documentation of education using OCH applications and technology.
• Coordinate transitions from hospital to home with facility staff, providers, and internal teams to ensure timely discharge and continuity of care.
• Provide in-person patient and caregiver education and gather insurance information for services initiation.
