Option Care Health, Inc. in Houston, TX seeks a Clinical Transition Educator to deliver clinical education and training that ensures smooth patient transitions to our home infusion model.

You will partner with the sales team, conduct patient assessments, educate caregivers, coordinate with facilities, and support timely discharges while maintaining confidentiality and documenting education in our systems.

Qualifications

• Active RN license required.
• Minimum of 2 years of healthcare experience.
• Strong communication and customer service skills.
• Proficient in MS Office applications (Word/Excel/PowerPoint).
• Willingness to travel locally to care facilities and partner sites.
• Ability to present and educate diverse audiences.

Responsibilities

• Educate patients, caregivers and facility staff on OCH services and home/alternate-site delivery.
• Conduct patient assessments to determine suitability for the care model.
• Coordinate with sales to identify educational needs and drive patient transition volume.
• Maintain confidentiality and comply with HIPAA and internal policies.
• Document education and interactions in OCH applications and systems.
• Facilitate hospital-to-home transitions with care teams to ensure timely discharge.
