Job summary

Option Care Health, Inc. is seeking a Clinical Transition Educator (RN) to provide clinical education and training that supports patient transitions to our home infusion care delivery model.

This role partners with the sales team to drive patient transitions, conducts assessments, and coordinates care transitions from hospital to home. The candidate will educate patients, caregivers, and facility staff, ensuring documentation and confidentiality while traveling to care facilities and partner

Qualifications

• Active and unrestricted RN license is required.
• Minimum of 2 years in healthcare.
• Willingness to travel to care facilities, hospitals, and partner sites.
• Excellent communication and presentation skills.

Responsibilities

• Educate patients, caregivers, and facility staff about OCH services and products for home or alternative sites.
• Coordinate with sales team to identify educational needs and improve patient transition volumes.
• Document education using OCH applications and technology with confidentiality and compliance.
• Support timely discharge, therapy education, and continuity of care from hospital to home.
