Job summary

Option Care Health is seeking a Clinical Transition Educator to deliver clinical education and training to ensure smooth patient transitions to our home infusion care model.

You will partner with the sales team to grow patient transitions, assess patient readiness, educate caregivers and facility staff, and document education using our systems. This role requires RN licensure, 2+ years healthcare experience, and strong communication.

Qualifications

• Active and unrestricted Registered Nurse (RN) license required.
• Minimum of 2 years of experience in the healthcare industry.
• Experience establishing and maintaining relationships with individuals at all levels of the organization.
• Experience applying knowledge of standard practices for all services offered (ACHC, URAC).
• Experience providing customer service to internal and external customers, including meeting quality standards of services.
• Basic PC skills: proficient with internet, email, Word, Excel, PowerPoint.
• Experience identifying operational issues and proposing improvements.
• Access to reliable transportation with valid driver's license and insurance.
• Willingness to obtain nursing licensure in additional states if needed.
• Able to plan, organize and make presentations.

Responsibilities

• Evaluate, educate, and train patients, caregivers, and facility staff about OCH services and products for in-home, alternate site, or virtual care.
• Conduct patient assessments to determine viability for OCH care delivery.
• Coordinate with the sales team to identify educational needs and improve patient transition volume.
• Maintain confidentiality of patient and proprietary information.
• Document education activities using OCH applications and technology.
• Coordinate transitions from hospital to home and other care settings with facilities and providers.
• Provide in-person patient and caregiver education and collect pre-admission and insurance information.
