Option Care Health is a leading home infusion provider, seeking a Clinical Transition Specialist to educate patients, families and facilities on our services and ensure successful transitions to home care. You will build and maintain relationships with hospital discharge teams and referral sources, coordinating with internal teams to optimize patient starts and outcomes.

Qualifications

• Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice.
• At least 2 years in the healthcare industry.
• Experience establishing relationships with individuals at all levels.

Responsibilities

• Proactively maintains and grows relationships with referral sources to increase sales and patient starts.
• Acts as key point of contact to educate and assist referral sources about Option Care services.
• Interacts with area providers to sell infusion services for discharging patients.
• Assesss patients selected by hospital to ensure viability for home health care.
• Educates patients and families about services and products offered by Option Care.
• Coordinate with internal teams to ensure timely discharge and treatment plans.
