Job summary

Pacific Health Group is seeking a Registered Nurse – Clinical Case Consultant to lead ECM care teams within CalAIM ECM. You will provide clinical guidance, supervise LVNs, and coordinate field-based, multi-disciplinary care for high-risk Medi-Cal members.

You will collaborate with Lead Care Managers, CHWs, physicians, and community partners to ensure safe, high-quality, member-centered services, while monitoring compliance and improving outcomes across hiring county.

Qualifications

• Active RN license in California.
• Minimum two years direct clinical experience in relevant settings.
• Experience with Medi-Cal or ECM preferred.

Responsibilities

• Provide clinical leadership and guidance to ECM care teams.
• Supervise LVNs and conduct field visits with care teams.
• Ensure documentation supports Medi-Cal ECM standards.
