Job summary

Pacific Health Group is seeking a Registered Nurse to serve as the clinical lead for CalAIM Enhanced Care Management (ECM), providing guidance to interdisciplinary teams and ensuring high-quality, member-centered care in a hybrid CA environment.

The role involves field visits, supervision of LVNs, collaboration with plans, hospitals, and community partners, and driving adherence to ECM standards while reducing avoidable utilization. Travel is required up to 50–70% within the county.

Qualifications

• Active RN license in California.
• Minimum two years of direct clinical experience in community health, managed care, acute/post-acute settings or behavioral health.
• Experience supporting Medi-Cal populations and complex social/medical needs is preferred.
• Ability to supervise LVN staff and coordinate interdisciplinary teams.

Responsibilities

• Serve as the primary clinical resource for ECM care teams.
• Provide clinical consultation to LVNs, CHWs, LCMs, and other team members.
• Lead field visits, coaching, and clinical oversight for adverse events and care transitions.
• Ensure documentation meets Medi-Cal ECM standards and quality measures.
• Collaborate with health plans, providers, hospitals, and community organizations.
