Job summary

Pacific Health Group is seeking a Registered Nurse – Clinical Case Consultant to lead clinical guidance for the CalAIM Enhanced Care Management program in a hybrid California environment.

You will supervise LVNs and CHWs, coordinate interdisciplinary care, ensure compliance with ECM requirements, and support high-risk Medi-Cal members across the county. The role requires field-based oversight, excellent documentation, and collaboration with partners.

Qualifications

• Active RN license in California.
• Minimum 2 years in community health or related settings.
• Experience with Medi-Cal populations and complex needs.
• Experience supervising LVNs or care teams preferred.
• CalAIM ECM knowledge or case management cert preferred.
• Willingness to travel up to 60-70% within the county.
• Bilingual skills preferred.

Responsibilities

• Provide clinical guidance to ECM care teams.
• Lead field-based supervision of LVNs and CHWs.
• Oversee care coordination and documentation quality.
• Ensure ECM requirements alignment and program compliance.
• Support transitions after hospitalizations and ED visits.
• Participate in quality improvement and staff development.
