Job summary

Pacific Health Group is seeking an experienced Registered Nurse to serve as the Clinical Case Consultant for our CalAIM Enhanced Care Management (ECM) program. The role provides clinical guidance, oversight, and support to interdisciplinary teams serving high-risk Medi-Cal members with complex needs.

The position requires California RN licensure, strong care coordination skills, and the ability to supervise LVNs and advance member-centered care across field and office environments.

Qualifications

• Active RN license in California.
• Minimum 2 years direct clinical experience in community health, managed care, acute care, post-acute care, public health, behavioral health, or SUD settings.
• Experience with Medi-Cal populations and individuals with complex medical, behavioral health, and social needs preferred.
• Strong communication, organizational, and problem-solving skills.
• Ability to work independently and collaboratively in a fast-paced environment.

Responsibilities

• Provide clinical leadership and consultation to ECM care teams.
• Guide care planning, interventions, risk mitigation, and treatment coordination.
• Participate in interdisciplinary case reviews and team meetings.
• Oversee field-based visits, shadow team members, and coach staff.
• Support LVN supervision and ensure compliance with regulatory standards.
• Coordinate care with primary and behavioral health providers, hospitals, and community organizations.
• Monitor documentation quality and ensure audit readiness.
