The ECM Clinical Consultant will act as a clinical resource for the care team, guiding assessment and care planning across physical, behavioral, and social needs.

Qualifications

• Licensed clinician with California licensure for professional services.
• Experience with Medi-Cal underserved and culturally diverse populations is preferred.
• Ability to oversee clinical assessments and care plans and provide guidance to the care team.

Responsibilities

• Oversee the clinical component of the ECM program and ensure high-quality care.
• Coordinate and participate in multidisciplinary team meetings.
• Ensure accurate documentation and alignment with health plan and DHCS requirements.
• Provide training and clinical guidance to ECM staff for assessments and care planning.
• Sign off on comprehensive assessments and care plans within required timeframes.
• Support care team in complex cases and facilitate access to primary and behavioral health services.
