Job summary

Qualifications

• Bachelor's degree required.
• BSN preferred.
• 5–7 years clinical experience in an acute care setting.

Responsibilities

• Collaborates with the multidisciplinary health care team to develop and coordinate the plan of care.
• Communicates with patient/family to identify and clarify goals and concerns.
• Performs pre-/post-admission assessments to identify high-risk patients and needs.
• Manages a caseload from admission through discharge and coordinates post-acute services.
• Ensures compliance with regulatory requirements and documents appropriately.
