Job summary

Johns Hopkins Medicine is seeking a Case Manager (RN) to coordinate patient-centered care from pre-admission through post-discharge, collaborating with patients, families, social workers, physicians, and the interdisciplinary team. You will lead transition planning, manage resources, and remove barriers to ensure timely and effective care.

The role requires a BSN, RN license, CPR, and 2+ years in clinical nursing with at least 1 year in case management.

Qualifications

• Completion of an accredited BSN Nursing Program (RN), required.
• Minimum 2 years of experience in clinical nursing required and minimum 1 year of experience in case management required.
• Valid state license as a registered professional nurse required.
• CPR- Basic CPR required.
• Certification in case management by a national accreditation organization i.e. - Registered Nurse (ACM - RN) / Certified Case Manager (CCM), preferred. Required to obtain within 1 year of hire.

Responsibilities

• Coordinate patient-centered care across the continuum and facilitate transitions.
• Lead and advocate for patients and families, coordinating resources and teams.
• Identify and remove barriers to delivery of clinical services with the care team.
