Job summary

Qualifications

• Bachelor's degree in Nursing (BSN) required.
• Minimum 2 years of clinical nursing experience.
• 1 year of experience in case management, discharge planning or utilization review preferred.
• Current valid state RN license and CPR certification required.

Responsibilities

• Lead and coordinate patient-centered care across the continuum to facilitate smooth transitions.
• Collaborate with patients, families, social workers and interdisciplinary team to identify and remove barriers to care.
• Manage resources and transition planning for an assigned caseload from pre-admission through post-discharge.
