Qualifications

ADN required; BSN preferred.
3-4 years clinical experience; 1-2 years case management preferred.
CCM certification preferred; state licensure required.

Responsibilities

Contributes to achievement of department goals and adheres to policies and safety standards.
Collaborates with multidisciplinary team to coordinate care plan.
Communicates with patient/family, care team, and external resources about goals and plan.
Develops and documents a comprehensive care plan with short and long term goals.
Evaluates response to care plan and recommends modifications as needed.
Maintains knowledge of regulatory requirements and reimbursement methods.
Manages caseload and identifies resources needed to implement plan.
Pursues ongoing professional growth in managed care and health care trends.
Directs decision making within plan of care and manages barriers to outcomes.
Participates in Performance and Service Improvement activities.
Performs comprehensive assessments to identify patient problems and needs.
