Responsibilities

• Contributes to department goals and adheres to policies, procedures, and safety standards.
• Collaborates with the multidisciplinary health care team to develop and coordinate the plan of care.
• Communicates with patient, family, and health care team to address issues and goals.
• Develops a plan of care with short- and long-term goals and timelines; involves family/caregiver.
• Evaluates patient response to care and suggests modifications; maintains accurate documentation.
• Manages caseload and identifies resources needed to implement the plan of care.
