Job summary

Requires a BSN, RN license and at least 2 years nursing experience with a minimum 1 year in case management; CPR certification is required, CCM/ACM certification preferred with a 1-year attainment window.

Qualifications

• Completion of an accredited BSN Nursing Program (RN) is required.
• Minimum 2 years of clinical nursing experience and at least 1 year in case management.
• Valid state RN license and CPR certification required; CCM/ACM preferred with 1-year to obtain.

Responsibilities

• Coordinate patient-centered care across continuum from pre-admission to post-discharge.
• Collaborate with patients, families, social workers and interdisciplinary team to remove barriers.
• Lead transitions planning and manage clinical resources within caseload.
