Responsibilities

• Collaborates with the multidisciplinary team to develop and coordinate the plan of care.
• Communicates with patients, families, and health care team to address goals and needs.
• Develops and monitors care plans across the continuum including discharge planning.
• Evaluates patient responses to the plan of care and recommends modifications as needed.
