Job summary

Responsibilities

• Contributes to department goals and adheres to policies, procedures, and safety standards.
• Collaborates with multidisciplinary team to coordinate plan of care.
• Communicates with patient/family and healthcare team to address issues and goals.
• Develops and documents a care plan with milestones and contingencies.
• Evaluates patient response and modifies plan as needed.
• Maintains knowledge of regulatory requirements and reimbursement methods.
• Manages caseload and identifies resources for care plan.
• Participates in performance and service improvement initiatives.
• Performs comprehensive assessments with the team to identify needs.
