Summa Health is seeking an RN Case Manager, Transitional Care at the Akron Campus to coordinate and lead patient care across the continuum from admission through discharge. You will develop, implement, and evaluate individualized plans with input from the care team, patients, and families to ensure safe, effective care and appropriate resource use.
