Job summary

Cleveland Clinic's Main Campus is seeking an RN Case Manager to collaborate with inter-professional teams and coordinate patient transitions across the continuum of care.

This full-time role supports comprehensive, individualized care planning, adherence to accreditation standards, and seamless handoffs to the next level of care. It follows a schedule of 8:00am to 4:30pm with 3 weekend shifts per quarter.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross.
• Three years of recent RN experience.

Responsibilities

• Collaborate with inter-professional teams across the continuum of care to coordinate transitions of care.
• Develop comprehensive, individualized transitional care plans with patient and family.
• Proactively assess and mitigate barriers to transitions of care.
• Lead patient rounding activities, huddles and committee meetings.
• Complete effective handoffs to the next level of care or community.
