Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross.
• Three years of recent RN clinical experience.

Responsibilities

• Collaborate with interprofessional teams to coordinate transitional care.
• Advocate for patients and conduct holistic assessments.
• Develop individualized care plans and empower self-management for patients and families.
• Support hospital throughput and discharge coordination.
• Provide post-discharge care coordination for safe transitions.
• Complete effective handoffs to the next level of care.
• Participate in patient rounding, huddles, and committee meetings.
