Job summary

Summa Health is seeking an RN Case Manager for Transitional Care in the Emergency department, 6:00pm-6:30am, at the Akron Campus. The role focuses on developing and coordinating individualized patient plans across the care continuum while leading the interdisciplinary team to ensure optimal outcomes and appropriate resource use.

The position requires an ADN with CCNE/ACEN-accredited program background, current Ohio RN licensure, and at least two years in case management or related fields.

Qualifications

• Must hold an RN license to practice in Ohio
• Two years of clinical/utilization management or case management experience required
• Graduate from CCNE/ACEN-accredited nursing program is required or preferred

Responsibilities

• Facilitates development, implementation, coordination, and evaluation of patient plans of care
• Collaborates with patient, family, physician, and interdisciplinary team to ensure care across the continuum
• Leads interdisciplinary team actions to optimize clinical outcomes and resource use
• Coordinates utilization review activities to ensure medically necessary care in appropriate settings
