Full Time, Monday-Friday

Summary

Facilitates development, implementation, coordination, and evaluation of a patient’s individualized plan triggered by referral from nursing, physician, interdisciplinary rounds, and other care team members. Collaborates with patient, family, physician, and interdisciplinary team to assure care is provided from acute care through the continuum. Includes taking a leadership role with the interdisciplinary team to achieve actions and processes designed to ensure optimal clinical outcomes, coordination, and continuity of health care as patients transfer between different levels of care and promotes effective utilization of health care resources through monitoring the patient’s hospitalization. Coordinates utilization review activities of assigned patients including reviewing and monitoring the utilization of health care services with the goal of maintaining quality, cost‑effective, medically necessary care in the appropriate setting.

Minimum Qualifications

Bachelor’s of Science in Nursing (BSN)
Graduate of a program of professional nursing accredited by the Commission on Collegiate Nursing Education (CCNE) or Accreditation Commission for Education in Nursing (ACEN)
Current license to practice registered nursing in the State of Ohio

Applicants with an Associate’s degree will be considered but are required to obtain BSN within three (3) years of employment.

Experience & Training Required

Two (2) years recent experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management that has provided a broad understanding in Case Management and Utilization Management concepts

Salary Range and Benefits

$37.40/hr – $56.11/hr. The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location, are considered when determining the starting rate of pay.
