Qualifications

• Minimum 5 years of nursing experience including leadership/management.

Responsibilities

• Develops and coordinates complex care plans for at-risk populations with the ID team.
• Directs patient through the system by coordinating care with physicians, staff, and outside facilities.
• Assesses patient needs and helps implement team recommendations.
