Job summary

Summa Health in Akron, OH is seeking an experienced nursing leader to develop, implement, coordinate, and evaluate complex care plans for at-risk patient populations. Working with the interdisciplinary team, you’ll guide patients through the system and coordinate care among physicians, staff, and external facilities.

You will assess patient needs, lead care plan implementation, and ensure delivery of high-quality, patient-centered care across Summa's network.

Qualifications

• BSN degree and Ohio license to practice
• CCNE- or ACEN-accredited nursing program
• 5 years in nursing with leadership/management experience

Responsibilities

• Develops, implements, coordinates and evaluates complex care plans targeting at-risk patient populations in consultation with the interdisciplinary team.
• Directs the patient through the system by coordinating care needs among physicians, direct care staff, interdisciplinary team and outside facilities/agencies.
• Comprehensively assesses the patient’s needs – and helps implement recommendations made by the team.
