Summa Health is seeking an RN Case Manager for Transitional Care in the Emergency department, 6:00pm-6:30am, at the Akron Campus. The role focuses on developing and coordinating individualized patient plans across the care continuum while leading the interdisciplinary team to ensure optimal outcomes and appropriate resource use.
The position requires an ADN with CCNE/ACEN-accredited program background, current Ohio RN licensure, and at least two years in case management or related fields.

Facilitates development, implementation, coordination, and evaluation of patient’s individualized plan triggered by referral from nursing, physician, interdisciplinary rounds, and other care team members. Collaborates with patient, family, physician, and interdisciplinary team to assure care is provided from acute care through the continuum. Includes taking a leadership role with the interdisciplinary team to achieve actions and processes designed to ensure optimal clinical outcomes, coordination, and continuity of health care as patients transfer between different levels of care and promotes effective utilization of healthcare resources through monitoring the patient’s hospitalization. Coordinates utilization review activities of assigned patients including reviewing and monitoring the utilization of health care services with the goal of maintaining quality, cost-effective, medically necessary care in the appropriate setting.
$37.40/hr - $56.11/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay.
