Job summary

Heart to Heart Hospice in Fishers, IN is seeking an experienced RN Case Manager to assess the terminally ill patients and develop an individualized care plan addressing physical, psychosocial, and environmental needs. You will coordinate with the patient, caregiver, and the IDG to ensure seamless care delivery.

Qualified candidates hold a nursing degree and current RN license, a valid driver's license with reliable transportation, and at least 2 years of RN experience in a healthcare setting.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License and reliable transportation.
• Two years of experience as an RN preferred in a healthcare/clinical setting.

Responsibilities

• Initial and ongoing assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized POC and revise the plan as necessary.
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meeting and coordinate the patient care planning process.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and established policies and procedures.
