Job summary

Heart to Heart Hospice is seeking a registered nurse to perform comprehensive assessments, develop and revise patient care plans, and collaborate with a multidisciplinary team. The role emphasizes delivering compassionate, in-home or facility-based palliative care while ensuring timely documentation and compliance with regulations.

The ideal candidate holds an RN license, has at least two years of clinical experience, and is willing to meet the job's travel and driving requirements.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Two years of RN experience preferred in a healthcare/clinical setting.

Responsibilities

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients' physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal.
• Implement the individualized POC and recommend revisions to the plan as necessary.
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings, coordinate and participate in the patient care planning process.
• Prioritize needs and collaborate with IDG to promote coordination of patient care.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation and ensure compliance with laws and policies.
