Job summary

Heart to Heart Hospice in Texas seeks an RN Case Manager to assess and coordinate care for patients with life-limiting illnesses, supporting families and guiding transitions.

You will develop and revise the plan of care, collaborate with IDG members, perform on-call duties, and ensure timely documentation while complying with regulations.

Strong bedside nursing background, current RN license, and eligible driver's license required; two years of RN experience preferred.

Qualifications

• Current State Registered Nurse License.
• Two years of RN experience preferred in healthcare/clinical settings.

Responsibilities

• Assess and address the impact of terminal illness on patients' physical, psychosocial, and environmental needs and ADLs.
• Implement and revise the plan of care as necessary.
• Collaborate with patient, caregiver, physicians and healthcare professionals.

What You Must Have:

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State Registered Nurse License
• Current Driver's License, reliable transportation,and provide ongoing valid and current auto liability insurance
• Two years of experience as an RNpreferred in a healthcare/clinical setting

What You Will Do:

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural and spiritual, verbal and non-verbal
• Implement the individualized POC and recommend revisions to the plan as necessary
• Collaborates care amongst patient/patient caregiver, physicians and healthcare professionals
• Attend Inter-Disciplinary Group (IDG) meeting, coordinate and participate in the patient care planning process, prioritization of needs and collaborates with IDG to promote coordination of patient care
• Perform on-call responsibilities and provide on-call service to patients/families as assigned
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
