Job summary

Home Health Care, Inc. is seeking an experienced RN Case Manager to oversee patient caseloads from initial home health assessment through recovery and discharge. You will coordinate care, maintain the clinical picture, and guide families and physicians through complex plans.

The role covers the Northwest Metro to St. Cloud corridor, with a full-time schedule Monday–Friday and one weekend per month. You’ll use OASIS tools and HCHB software daily to ensure high-quality patient outcomes.

Qualifications

• Excellent clinical judgment and critical thinking.
• Strong organization and time-management abilities.
• Excellent physician, patient, caregiver, and interdisciplinary communication.
• Ability to work independently in a field-based environment.

Responsibilities

• Managing an assigned home health patient caseload.
• Completing OASIS Start of Care, Recertification, and Discharge assessments.
• Performing comprehensive nursing assessments.
• Developing and updating individualized plans of care.
• Providing skilled nursing visits.
• Monitoring patient progress throughout the episode of care.
• Recognizing changes in condition and intervening appropriately.
• Providing medication management and patient education.
• Performing wound care and other skilled nursing services.
• Communicating with physicians regarding clinical concerns and changes.
• Coordinating with PT, OT, SLP, social work, and other interdisciplinary team members.
• Educating patients and caregivers throughout recovery.
• Helping prevent avoidable emergency department visits and rehospitalizations.
• Completing timely documentation using Homecare Homebase (HCHB).
