Job summary

Home Health Care, Inc. is seeking a Registered Nurse (RN) Case Manager to oversee a home health caseload across Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.

You will perform OASIS assessments, develop care plans, and deliver skilled nursing visits with a strong emphasis on patient education. The role offers a 8:00 AM–5:00 PM schedule, a Monday–Friday workweek with 1 weekend per month and 1 on-call shift per week, and a competitive compensation package including bonuses.

Qualifications

• Active Minnesota RN license.
• 3+ years RN experience, home health preferred.
• Strong clinical judgment and ability to work independently.
• Excellent organization and communication skills.

Responsibilities

• Manage a home health caseload across Woodbury, Cottage Grove, Hastings, St. Paul Park, and Landfall.
• Complete OASIS assessments and individualized plans of care.
• Provide skilled nursing visits, assessments, and patient education.
• Coordinate care with physicians and the interdisciplinary team.
• Help patients remain safe and independent in their homes.
• Maintain accurate and timely clinical documentation.
