Job summary

Home Health Care, Inc. is seeking an experienced RN Case Manager to oversee a Northwest Metro to St. Cloud home health caseload. You will coordinate care from initial assessment through recovery and discharge, building comprehensive plans and ensuring quality outcomes.

The role emphasizes independent field work, collaboration with therapists and social work, and use of Homecare Homebase (HCHB) for documentation. Excellent clinical judgment is essential.

Qualifications

• Active Minnesota RN license.
• Three or more years of RN experience.
• Home health experience preferred.
• OASIS experience highly valuable.
• Excellent clinical judgment and critical thinking.
• Strong organization and time-management abilities.
• Ability to work independently in a field-based environment.

Responsibilities

• Coordinate and oversee an assigned home health caseload.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Monitor patient progress throughout the episode of care.
• Recognize changes in condition and intervene as needed.
• Provide medication management and patient education.
• Perform wound care and other skilled nursing services.
• Communicate with physicians regarding clinical concerns and changes.
