Home Health Care, Inc. is seeking an RN Case Manager to oversee a focused Southwest Minneapolis & Inner Metro caseload. You will coordinate with physicians, therapists, and families while delivering skilled nursing and managing documentation. You’ll handle admissions through discharge and ensure optimal patient progress.

The role emphasizes ownership of the patient journey, ongoing education, and seamless transitions home with strong clinical judgment and independence.

Qualifications

• Active Minnesota RN license required.
• Three+ years of RN experience.
• Home health experience preferred.
• OASIS experience highly valuable.
• Strong assessment and critical-thinking skills.
• Reliable transportation for visits to your territory.

Responsibilities

• Manage an assigned home health patient caseload.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Perform comprehensive nursing assessments and develop care plans.
• Provide skilled nursing visits and monitor patient progress.
• Educate patients and families on diagnoses, medications, and treatments.
• Coordinate with physicians, therapists, and interdisciplinary team members.
• Ensure timely documentation using Homecare Homebase (HCHB).
• Assist patients with transitions from hospital to home.
