Job summary

Home Health Care, Inc. seeks an experienced RN Case Manager to oversee a patient caseload in the Minnesota home health setting. You will guide patients from initial assessment through recovery and discharge, coordinating with interdisciplinary teams and families to ensure seamless care.

You will manage ongoing patient progress, document thoroughly, and use clinical judgment to adapt plans as needed. This role blends hands-on nursing with leadership in care coordination.

Qualifications

• Active Minnesota RN license is required.
• Three or more years of RN experience are required.
• Home health experience is preferred.
• OASIS experience is highly valuable for this role.

Responsibilities

• Coordinate and oversee an assigned home health caseload.
• Complete Start of Care, Recertification, and Discharge assessments (OASIS).
• Develop and update individualized plans of care.
• Monitor patient progress throughout the episode of care.
• Recognize changes in condition and intervene appropriately.
• Provide medication management and patient education.
• Perform wound care and other skilled nursing services.
• Communicate with physicians about clinical concerns and changes.
• Coordinate with PT, OT, SLP, social work and other team members.
• Educate patients and caregivers throughout recovery.
• Complete timely documentation using Homecare Homebase (HCHB).
