Job summary

Home Health Care, Inc. is seeking a Registered Nurse (RN) Case Manager to oversee patient care in the St. Paul service area. You will lead nursing visits, coordinate with physicians and therapists, and ensure safe, effective home health outcomes.

Qualifications

• Active Minnesota RN license is required.
• Three or more years of RN experience.
• Home health experience preferred.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and leadership abilities.

Responsibilities

• Manage a home health patient caseload throughout the St. Paul service area.
• Complete OASIS assessments and develop individualized care plans.
• Perform skilled nursing visits and comprehensive clinical evaluations.
• Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
• Provide medication management, disease education, and ongoing patient support.
• Monitor patient progress and adjust care plans as needed.
• Help patients remain safely at home while reducing avoidable hospitalizations.
