Job summary

Home Health Care, Inc. is seeking an RN Case Manager to oversee a Northwest Metro to St. Cloud patient caseload, coordinating care from initial home health assessment through discharge.

You will complete OASIS assessments, develop and update care plans, and monitor patient progress while collaborating with physicians and a multidisciplinary team. Documentation occurs in Homecare Homebase, with a Monday–Friday schedule and occasional on-call duties.

Qualifications

• Active Minnesota RN license required.
• 3+ years RN experience in nursing; home health preferred.
• OASIS experience and care coordination.
• Strong clinical judgment and communication skills.

Responsibilities

• Coordinate and oversee an assigned home health caseload.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Monitor patient progress through the episode of care.
• Communicate with physicians about clinical concerns and changes.
• Coordinate with PT, OT, SLP, social work, and other team members.
• Educate patients and caregivers throughout recovery.
• Document using Homecare Homebase (HCHB).
