Job summary

Home Health Care, Inc. is seeking an experienced Registered Nurse (RN) Case Manager to manage a home health caseload across St. Paul and surrounding areas.

You will complete OASIS assessments, develop plans of care, deliver skilled nursing care, and collaborate with physicians and an interdisciplinary team. We offer a Monday–Friday schedule, weekend on-call coverage, competitive compensation from $85,000 to $130,000 plus $6,000 in first-year bonuses, mileage and licensure reimbursements, and

Qualifications

• Active Minnesota RN license.
• 3+ years RN experience (home health preferred).
• Strong clinical judgment and critical thinking skills.
• Excellent communication and organizational abilities.
• Reliable transportation for local travel.

Responsibilities

• Manage a home health caseload across St. Paul, with on-call and weekend visits within ~35 miles of home.
• Complete OASIS assessments and individualized plans of care.
• Deliver skilled nursing care and patient education.
• Collaborate with physicians and interdisciplinary team members.
• Help patients recover and remain independent at home.
• Maintain accurate, timely clinical documentation.
