Job summary

Home Health Care, Inc. is seeking an RN Case Manager to oversee a home health caseload in Edina and surrounding southwest Minneapolis communities. You’ll perform OASIS assessments, develop plans of care, and coordinate with physicians and therapists to move patient care forward.

This full-time role offers a structured schedule, on-going professional autonomy, and a strong team behind you. You will educate families, monitor progress, and proactively adjust care plans to prevent hospitalizations.

Qualifications

• Active Minnesota RN license (or compact state with MN license transfer).
• Three or more years of RN experience.
• Home health experience preferred; OASIS experience highly valuable.
• Strong assessment, critical-thinking, and time-management skills.

Responsibilities

• Oversee assigned home health caseload from admission to discharge.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Monitor patient progress and coordinate with physicians and therapists.
• Perform wound care, medication management, and other skilled nursing services.
• Educate patients and families on diagnoses, medications, and treatments.
