Job summary

PruittHealth Hospice Macon, GA is hiring a Registered Nurse Case Manager (RN) to deliver compassionate hospice care in patients’ homes or care facilities. You will assess needs, coordinate care plans, and maintain accurate documentation per policy to ensure continuity of care.

Qualifications

• Current, active RN licensure in the state of practice.
