Job summary

PruittHealth Hospice in Georgia is seeking a Registered Nurse Case Manager (RN) for on-call weeknights to join our specialized hospice team. Provide home or facility-based care with a patient-centered approach, ensuring quality and continuity of care.

Qualifications

• Current, active, and unrestricted RN licensure in the state of practice (Georgia).
• Hospice/home health care experience is a plus.

Responsibilities

• Complete evaluations, assessments, and treatments per the individualized care plan.
• Complete documentation in a timely and accurate manner.
• Maintain continuity of patient care in a hospice setting.
• Follow established policies and procedures related to the scope of practice.
