Job summary

PruittHealth Hospice Macon in Georgia is seeking a compassionate Hospice Admissions RN to evaluate and admit patients to our hospice program. You will coordinate with families and care teams to ensure seamless transitions and compliant care planning in home or facility settings.

Responsibilities include conducting assessments, documenting care plans, and maintaining continuity of care while adhering to hospice policies.

Qualifications

• Current, active RN licensure in the state of practice.
