Job summary

PruittHealth Hospice Macon in Georgia is seeking a Registered Nurse Case Manager (RN) to join our hospice team delivering compassionate in-home and facility-based care. You will coordinate patient assessments and care plans.

This role emphasizes thorough documentation, adherence to policy, and ensuring continuity of care across shifts. We offer top‑of‑market pay, comprehensive health plans, and flexible scheduling.

Qualifications

• Current, active, and unrestricted RN licensure in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.

• Maintain continuity of patient care in a hospice setting.
