Job summary

PruittHealth Hospice Blue Ridge in Georgia is seeking a Registered Nurse Case Manager to join a dedicated team delivering compassionate hospice care in patients’ homes or care facilities. This full-time, first-shift role focuses on coordinating clinical care and supporting families with dignity and respect.

You will complete evaluations and treatments per the care plan, maintain timely documentation, follow policies, and ensure continuity of patient care.

Qualifications

• Current, active RN licensure in the state of practice.
• Experience in hospice care preferred.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
