Job summary

The Texas Health and Human Services Commission (HHSC) Richmond State Supported Living Center is seeking an experienced Registered Nurse Case Manager to provide complex clinical leadership and nursing care for residents with medical and behavioral needs. This role emphasizes coordination with the IDT, LARs, and direct care staff to implement tailored care plans and ensure high-quality outcomes.

Qualifications

• RN license in Texas (or eligible)
• Experience in case management preferred
• Strong communication and documentation skills
• Ability to work with individuals with complex medical needs

Responsibilities

• Performs highly complex nursing case management for assigned individuals, including those with complex medical and behavioral health needs.
• Develops, evaluates, and monitors nursing interventions to ensure treatment plans and quality care.
• Coordinates admission activities and transports, and participates in specialized clinics and ISP processes.
• Trains and coaches staff on nursing practices and case management procedures.
• Maintains required nursing documentation and supports continuity of care across teams.
